Avez-vous fait votre valise vous-même ?
Have you (formal/plural) packed your suitcase yourself?

shouldn't"fait" be "fais"from tu/vous fais?

French is so complicated!  Really, you, your and you (rself)  in the same short sentence.

In this case, the passé composé is being used so the structure is verb [être or avoir] which is conjugated +  participle [in this case, the participle of faire is fait]. With avoir, the participle is invariable, so you're only conjugating avoir.

Tu as fait ... / As-tu fait... ?
Vous avez fait... / Avez-vous fait

Ex: You said... / Did you say... ? {participle of dire->dit}
Tu as dit... / As-tu dit... ?
Vous avez dit... / Avez-vous dit ?
