The way I understand it "¿cómo estoy" means "How am I".
How would this phrase be used in conversation?
¿cómo estás ("How are you") is obviously useful but I wonder why "¿cómo estoy" is in the first lesson.

Hi David, It is used in lesson one as a question. Probably to reiterate just being asked how are you. Something like..... How are you? How am I? I am fine, thanks. I doubt it is used much. Hope this helps

In lesson one I expected common usage so it through me a little.

Since it is used in the practice portion, it appears to be just an example and not a normal greeting in itself. Often things in the practice sections are just snippets of conversations.
How are you?
fine, and you?
How am I? I'm fine!

Glad I wasn't the only one wondering when "How am I" comes up in a conversation. lol
